1 Approved JBVAMC Protocol and JBVAMC IACUC approval letter must be submitted with Form H. Forms A and B, and Appendices 1 and 2 are not required for this type of submission. Complete submissions are due no later than Wednesday preceding UIC ACC meeting.
2 Appendices 1, 2, or Biologic materials are not required for modifications if they were submitted with the same species/strain of animals for breeding or the same hazardous agent or biologic material used in the same manner with the original protocol or a formerly approved modification.
