New guidance for formatting of recruitment and consent documents
Effective June,1, 2023, the UICIRB stamp will be moved from a horizontal orientation in the upper right corner of documents to a vertical orientation in the upper third of the left side margin.
To ensure that important text is not covered by the stamp, please use the following guidelines when formatting your informed consent documents (including information sheets and scripts) and recruitment materials (e.g., flyers, emails, websites, etc.):
- The left side margin should be set at 1.25 inches and the top margin set at 1 inch; OR
- a rectangular area of 2 inches tall by 1.25 inches wide is to be left blank in the upper third of the left side margin.
- Documents should be in WORD or PDF formats only. Other formats may not be able to be stamped by UIC Research.
